EXPLANATORY FACTORS FOR NON-COMPLIANCE TO ANTI-RETRO VIRAL TREATMENT AMONG PEOPLE LIVING WITH HIV AND UNDER TREATMENT IN THE KARISIMBI HEALTH DISTRICT, IN THE DEMOCRATIC REPUBLIC OF CONGO

Introduction: Knowledge of the factors favorable to treatment compliance among patients suffering from HIV/AIDS remains a concern for all health professionals. This study aimed to determine the level of compliance with antiretroviral treatment (ART) and associated factors among people living with human immunodeficiency virus (PLHIV) treated in the Karisimbi health zone.
Methods: this was an analytical cross-sectional study carried out in the Karisimbi health zone from October 1 to December 31, 2021 with a simple probability sample of 384 people living with HIV under anti-retroviral treatment in the period from August 1, 2020 to September 30, 2021 in three target care structures in the health zone, North Kivu, in the Democratic Republic of Congo. The data were analyzed with SPSS 21.
Results: The prevalence of non-compliance with ARV treatment was 24.7%, the median age was 39 years (interquartile range of 7 years) and the female gender was the most represented (sex ratio: 3.5). The factors significantly associated with non-compliance were: home time – care structure (ORa: 1.7; 95% CI: 1.07 - 2.69); stigma (aOR: 1.6; 95% CI: 1.01 - 2.82); not belonging to a support group (ORa: 0.5; 95% CI: 0.27 - 0.94) and the use of an alternative treatment to ARVs (ORa: 2.3; 95% CI: 1 .30-4.09).
Conclusion: It appears in this study that the prevalence of non-compliance with anti-retroviral treatment in this health zone was high. We recommend promoting the education of PLHIV through therapeutic education sessions with a view to improving compliance with ARVs in order to reduce the risk of treatment failure.
